Under general supervision of the Supervisor, APC Care Management, the APC Administrator and the Ambulatory Site Clinical Lead provider (MD/DO, APRN, PA), perform as a member of the healthcare team to apply the nursing process when making nursing judgments and decisions specific to assessment, planning, implementation/delivery, and evaluation of patient care, and documenting relevant data and care processes. The RN Care Manager will facilitate communication between the patients and their Primary Care Providers. In addition, the RN Care Manager will assist with chronic disease action plans, transitional care management, Advance Directives planning, Annual Wellness Visits, and preventative care utilizing assessment skills both in person and telephonically. What You Will Do
- Exercise professional nursing judgment appropriate to the level of care needed, and prioritizing patient acuity and workload demands. The RN Care Manager will function with greater autonomy by consistently demonstrating independent clinical decision making abilities.

- Coordinate continuity of care with patients and families following hospital admission, discharge and ER visits
- Advocate for patients by assisting in navigation of the health system
- Communicate to the care team the patient’s goals and progress
- Participate in quality improvement initiatives
- Participate on data collection team to evaluate health outcomes, trends, utilization, cost reductions, and programmatic evaluation for areas of improvement
- Provide 1 on 1 office visits for patients to implement care planning

Family Medicine Consultant will provide comprehensive primary healthcare physically and through telemedicine to individuals and families of all ages, encompassing preventive, acute, and chronic care. Will play a crucial role in diagnosing and treating a wide range of medical conditions, promoting health education and preventive measures, and coordinating care with specialists and other healthcare professionals. Additionally, you will be responsible for practicing population health management and value-based care, focusing on improving patient outcomes through efficient and effective healthcare delivery through specific care programs and clinical pathways, with an emphasis on delivering patient-centered care and optimizing health outcomes
Patient Consultations and Management
- Conduct comprehensive telehealth consultations and in-person consultations via thorough patient history and physical examinations with patients experiencing Family Medicine and general medicine complaints
- Provide healthcare to all family medicine target population across age and gender
- Provide chronic diseases and preventive care consultations and follow up to patients as per clinical guidelines
- Utilize video conferencing platforms to assess patient symptoms, review medical history, and perform virtual examinations. Develop personalized treatment plans, prescribe medications, and provide ongoing patient education
- Deliver high-quality medical consultations to patients
- Utilize telehealth platforms for virtual appointments and conduct in-person assessments as needed.
- Develop and implement personalized treatment plans, including medication management, referrals to specialists, and lifestyle modifications
- Oversee ongoing care for chronic and acute conditions
- Adjust treatment plans as necessary based on patient feedback and clinical outcomes.
- Collaborate with other healthcare professionals, including primary care physicians, specialists, and nurses, to ensure seamless patient care
- Practice population health management by identifying and addressing health issues at the community level, implementing preventive measures, and promoting health education and awareness

- Maintain up-to-date knowledge of telehealth regulations and best practices. Participate in quality improvement initiatives and document patient encounters accurately

- Maintain accurate and complete patient records in accordance with HIPAA regulations

- Actively participate in training programs and stay updated on platform enhancements and best practices. Collaborate with IT support to troubleshoot any technical issues
Patient Safety and Satisfaction
- Maintain compliance with all patient safety standards and ensure patient safety across all clinical interactions
