**Essential Responsibilities and Duties**:
1.Collects data from medical records and other sources, analyses data, prepares and presents reports for the assigned section inclusive of summary of the information, trending of the data with graphical presentation and recommendations.
2.Participates in PI teams for PI projects and activities. Validates and documents the accuracy of collected data for (PI) activities and for decision support data.
3.Process incidences reported in the Safety Reporting System (SRS), and performs required investigations including the conduction of root cause analysis of actual and potential serious/sentinel events in the Hospital.
4.Maintains databases related to the assigned sections.

**Education**:
Bachelor’s or Associate Degree/Diploma in Nursing, Quality or healthcare related discipline is required.
**Experience Required**:
Four (4) years of healthcare related experience in a tertiary facility with Bachelor’s or six (6) years with Associate Degree/Diploma is required.
**Other Requirements(Certificates)**:
- Certified Professional in Healthcare Quality (CPHQ), Certified Professional in Healthcare.
- Risk Management (CPHRM) or other related Healthcare Quality certificate is preferred.

Summary
