Monitor, review and update patient’s record files both in-patient and out-patient to ensure the completeness and proper documentation are observed with the accordance to both local and international standards.
Key Responsibilities / Accountabilities
- Review medical records from departments such as Emergency Department (ER), In-patient and Out-patient departments based on review requirements using established international and local standards.
- Organize and prioritize work to ensure that all review documents are completed in accordance with the pre-set timelines.
- Work in collaboration with Medical Administration and Quality Department to ensure efficient records reviews are complete.
- Review prior discharge of patient files for the completeness and by assuring the accuracy of a Medical Records.
- Follow-up of incomplete documents by calling the concern physicians (consultants, specialists and residents).
- Coordinate in-service meetings to educate medical staffs on the proper documentation procedures and repot any incorrect or missing documents to the concerned physician and/or HOD for the completion.
- Actively participates in Quality meetings/ reviews as required to certify adherence to established concrete guidelines.
- Prepare and present monthly report in Quality Meetings and Surveys.
- Prepare a regular meeting to the physicians known to have deficient medical files for education.
- Orienting newly hired physicians for all related issues in the department i.e. VIDA System, Approval and Claims.
- Assure the full representation of clinical pathway in all file compartments.
- Comply with the standards laid down by the Joint Commission International Accreditation (JCIA) / Central Board for Accreditation of Healthcare Institutions (CBAHI).
- Follow and apply medical records department policy and procedure and insures that other staffs were following it.
- Report to Medical Coordinator for any violation of Medical Records Documentation (MRD) and Policies and procedures (P&P).
- Adhere the safety and security measures and apply the infection control measures.
- Participate in the hospital education activity program.
- Participate in, and lead person-centered care initiatives undertaken by HMG.
- Enrich patient experience with compassion, respect and dignity.
- Perform all other tasks/duties assigned within the realm of his/her knowledge, skills and abilities.
Education/ Professional Qualification
- Bachelor’s Degree
- Professional - Nursing; preferably Medicine and Surgery
- Preferably four (4) years’ experience in a medical records environment.

**Essential Responsibilities and Duties**:
1. Prepares Medical Records for new patients by creating bar code label and assigning proper color code.
2. Receives discharge patient charts from all wards and matches with the discharge list.
3. Verifies all discharge papers belong to the same patient by Medical Record Number.
4. Assembles the records by following approved chart order.
6. Retrieves incomplete records for physicians. Accurately performs second analysis and audits incomplete records ensuring compliance with record completion.
7. Performs thinning of records based on the length of stay for the patients.
8. Files medical records in Terminal digit order and loose sheet documents in correct patient charts and in correct chart order.
9. Answers phone calls, greets walk-in customers, retrieves patient information from ICIS System and takes necessary action as needed.
10. Retrieves scheduled patient charts by clinics, transfers and confirms the records using chart tracking system, delivers and collects records to/from the clinic.
11. Purges medical records by utilizing the report on the discharged patients and notifies the concerned Department.
13. Participates in the orientation of new employees as required.
14. Ascertains cross-training to float into other positions within HIM.
15. Maintains the incomplete chart area including coordinating activities of the physicians' notices complying with JCIA standards as the a I to incomplete records.
16. Process transcribed reports and provides support services to the transcription area.
17. Performs purge/audit and maintains assigned digits.
18. Follows all Hospital related policies and procedures.
19. Participates in self and others' education, training and development, as applicable.
**Education**:
Bachelor's or Associate Degree/Diploma in Medical Records Management or other Medical Records related discipline is required.
**Experience Required**:
One (1) year of related experience with Bachelor's Degree or three (3) years with Associate Diploma is required.

Summary :
The Medical Assistant role provides clinical and administrative support for patients under the direction of a Provider and/or licensed Nurse.
Responsibilities :
- Prepares patients for examination; takes and records vital signs; performs phlebotomy as needed; administers medications as directed by the Providers according to policy and procedure
- Collects data that contributes to the assessment and evaluation of individualized care and needs of assigned patients, including discharge plans, under the direction of the Licensed Nurse
- Collects, processes, and submits laboratory specimens in accordance with policy and procedure
- Communicates patient findings and pertinent information to the Provider and/or Licensed Nurse utilizing appropriate communication/documentation processes
- Accurate and timely documentation of care within EPIC system
- Performs point-of-service activities such as appointment scheduling, visit preparation, co-payment collection and patient tracking as needed
- Performs clinical duties that are department specific such as POCT, hearing and vision screenings, and any other procedures
- Performs routine task and general office duties such as faxing, copying, filing, etc. as needed
- Cleans, stocks, and prepares examination rooms
- Develops and maintains positive relationships with patients, families, and colleagues
- Follows safety policies and procedures in the delivery of care to assure a safe environment for patients, families, and other staff members

Technical Expertise
- Experience in phlebotomy is preferred.
- Knowledge of and experience in medical terminology is required.
- Experience working in healthcare environment is preferred.
- Experience working in Microsoft Office (Outlook, Excel, Word) or similar software is required.
- Experience working in electronic medical record (i.e., EPIC) or similar software is preferred.
Education and Experience
- Education: High School Diploma or equivalent is required.
- Completion of an approved (valid) Medical Assistant Program, EMT Program, or Paramedic Program is required.
- Certification:Basic Life Support (BLS) training from the American Heart Association is required.
- Certification: Medical Assistant certification preferred.
- Years of relevant experience: no experience required.
