Manages, organizes andcoordinates the administrative and operational activities in Case Management(CM). Develops proposals and recommends improvements and modifications toenhance patient experience and maximize the utilization of all Ambulatory Careareas. Establishes and maintains communication and collaboration with differentservices to ensure the delivery of high quality care to patients.
**Essential Responsibilities and Duties**:
1. Managesthe day-to-day activities of the section, assignsresponsibilities and tasks to staff and evaluates performance.
2. Preparesand recommends the section plans and organizational changes related to thepatients’ admission.
3. Coordinates,monitors, and evaluates options and services provided by the Section, andfacilitates the treatment and support to the patient and the patient careproviders.
4. Directsand monitors the activities of CM staff to ensure that patient admission,discharge, post Hospital care are completed in an effective and efficientmanner.
5. Establishes report mechanismsto improve business processes, patient satisfaction, efficiency andproductivity of staff.
6. Prepares and submitsoperational statistics reports, and acts as a resource person of CM asrequested.
7. Solvesidentified problems in regards the quality and appropriateness of inpatientcare through direct coordination and resolution of issues with relevantdepartments.
8. Providesthe administration with statistical reports, studies, and analysis on allpatient related matters, as directed.
9. Maintains an effective communication with Medical Departments, ClinicalServices, Nursing Affairs and other relevant departments to coordinate andintegrate all services in the inpatient, streamline the patient flow andenhance patient experience.
**Education**:
Master’s or Bachelor’s Degreein Hospital Administration or other related discipline isrequired.
**Experience Required**:
Five (5) years of related experienceincluding two (2) years at a Supervisory/Senior with Master’s, or seven (7)years with Bachelor’s Degree, position is required.
**Other Requirements(Certificates)**:
Saudi Nationals Only.

Duties and Responsibilities
- Provide personalized coordinated care, and support for patients and families.
- Serve as the advocate, liaison, and resource for patients/family.
- Assess patient's health and social needs.
- Orient and educate patient/family to the current condition.
- Develop a care plan and set goals for best practices involving patient/family.
- Monitor patient progress in a timely manner, and facilitate changes as needed.
- Ensure sufficient time is given to address all patient/family needs and answer all concerns.
- Increase patients’ ability for self-management and shared decision making.
- Collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
- Involve patients, families, and caregivers by providing them with education and resources for optimal support to promote recovery.
- Conduct educational need assessment for patient/family by assessing learning needs, literacy level, readiness, and ability to learn, and choose the best suitable learning method.
- Provide education to patient/family in an easy-to-understand language.
- Provide patient/family with educational material that meets their learning skills.
- Create and promote adherence to a care plan, developed in coordination with the patient, patient family and primary care provider.
- Assess, arrange, and educate who will provide care after discharge to maintain continuity of care and maintaining data.
- Maintain patient database and enter new patient information as needed.
- Responsible for the appointments of visiting doctors.
- Ensure the follow-up appointments are attended by patients.
- Collaborate and educate patients about external community resources as needed.
- Educate patient how and when to access emergency services when needed.
- Attend promptly to telephone calls via official mobile or department phones.
- Reduce room utilization, emergency visits and hospital readmissions.
- Facilitate the visit of Walk-in patients.
- Treat people with dignity, compassion, and respect and follow patient and family rights and responsibilities.
- Adhere to ASH policies.
- Diploma degree from medical or nursing school or any equivalent.
- Arabic is essential, English is an assist.
- BLS certified
- At least 2 years of healthcare experience in a hospital.
- Ability to work independently and facilitate teamwork within the department and with others in the hospital.
- Excellent verbal and written communication skills.
- Excellent computer skills with desire to increase efficiency by using all available technology.
- Ability to coordinate, track and effectively communicate with clinical and non-clinical staff.
