Description
Job Duties and Responsibilities:
- Address social concerns of patients.
- Perform psychosocial assessments and counseling therapy for patients and their families.
- Provide follow-up services to appropriate patients and families.
- Participate in preparing treatment plans, especially for patients with psychosocial issues requiring resources such as family support, transportation, and financial assistance.
- Assist in preparing discharge plans to help patients return to their home environment with optimal functioning.
- Respond to referrals for inpatients, outpatients (RDU, special needs, kidney transplant patients), and ER, as per policy.
- Follow up on patients after initial assessment when indicated.
- Support families coping with loss and grief (Code Mercy and Code Blue).
- Provide support to families of DNR (Do Not Resuscitate) patients.
- Assist patients facing financial needs by creating case studies and referring them to appropriate charity channels.
- Contact patients who refuse admission or continuation of care to ensure stability and understand reasons for DAMA, as per policy.
- Provide special care for patients with specific needs, such as terminally ill, handicapped, victims of abuse or domestic violence, and those with social problems not psychiatric in origin.
- Coordinate reporting of abuse and domestic violence cases to MOH and HRSD, and report to the Violence and Abuse Protection Committee.
- Participate in multidisciplinary discharge planning teams.
- Coordinate visits of charity institutions to inpatients periodically.
- Issue Priority to Them Cards and parking stickers for special needs patients, as per policy.
- Assist in communication with clergy and interpreters for patients, if needed.
- Coordinate with the accounting department regarding financial coverage and limits.
- Attend relevant committees and meetings as recommended.
- Perform other duties within the scope of responsibility and requirements of the job.
- Arrange and attend family/physician meetings, including preparation.
- Provide screening and support to patients and families in ICU, CCU, NICU, and PICU.
- Maintain an updated list of interpreters, clergy, and sign language services for patient communication.
- Maintain extensive information on community resources and programs to meet patient needs and share this information with patients and families.
- For kidney transplant social workers: conduct psychosocial evaluations, present data, address problems to the transplant committee, and educate patients on post-transplant expectations regarding medications and insurance.
- Demonstrate compassionate communication skills, ensuring patient-centered care.
Other Job Functions:
- Perform all duties in line with Person-Centered Care values.
- Adhere to all policies, plans, and programs within Fakeeh Care.
- Follow the code of conduct and ethical standards outlined in HCM policies and guidelines.
- Complete mandatory education at least one month prior to expiration.
- Maintain up-to-date knowledge on quality, safety, accreditation, policy management, and document systems.
Qualifications:
- Bachelor's degree in Sociology or Psychology.
- Minimum of 2 years full-time experience as a social worker.
- Valid Saudi Council Certification.
