3. Maintains liaison with referring hospital and referring physicians to ensure that all necessary information and documentation regarding patients are available.
4. Manages and tracks the daily referrals of new patients.
5. Monitors the initiation and follow up of the discharges planning process.
6. Ensures that Case Management Assessment is done within one working day admission and documented within 48 hours.
7. Ensures that unit rounds are conducted on a daily basis.
8. Makes recommendations regarding the admission/ appointments process and the effective use of bed and resource utilization.
9. Communicates, documents and reports all relevant information/ statistics as per established standards.
10. Maintains positive professional interpersonal relationships with patients and theirfamilies, co-workers and other members of the Health Care Team.
11. Assists in preparation of performance appraisals.
12. Addresses program related matters, advising and guiding staff, including the development and implementation of program policies, procedures and protocol and budget preparation, consults with Case Manager or Executive Assistant as appropriate.
**Education**:
Bachelor’s Degree or IPA Diploma in Hospital Administration or other related discipline is required.
**Experience Required**:
Three (3) years of related experience with Bachelor’s Degree or five (5) years with IPA Diploma is required.

**Position** **Purpose**
The **Patient** **Support** **Nutritionist** provides comprehensive Patient care in the community setting that assists patients to achieve optimum health and quality of life in situations of chronic disease. The Patient Support Nutritionist is responsible for the provision and management of effective Patient care to individuals and/or families in the community or in-Patient Support Programs. **Duties and Responsibilities**
Based on the agreed-upon services and workflow, the Patient Support Nutritionist provides skilled Patient services in accordance with the orders of a physician and under a plan of care. Duties include but not limited to:
- Telephonic follow up consultation
- Ensures clients’ menu selections are in compliance with prescribed diet order, nutritional adequacy, age, cultural background, and religious practice.
- Writes and modifies menus as needed according to client diet prescription using established resources/guidelines.
- Updates, monitors, and follows-up on all pertinent client information like diet order changes, food allergies, food preferences, and requests.
- Records all assigned data such as client meal census and floor stock requests on paper logs or spreadsheet.
- Resolves client requests and complaints or relays to appropriate staff.
- Serves as a dietary resource for nursing and other staff.
- Provides general information on diet utilizing approved materials and resources.
- Education and counselling, consistent with the type/nature of treatment and/or care management need.

Educational Background and Experience
- A BS Degree in Nutrition/ Dietitian is required with at least 2 years working experience in a healthcare setting

- The position entail frequent travel to Clients, partners and Pharmacies which may be outside your designated region.

5. Responds to patient inquiries and complaints by directing the issues to the concerned department and/or appropriate representative to ensure a satisfactory solution, ensuring patient satisfaction.
6. Develops and recommends new methodologies. Maintains the section policies and procedures.
7. Participates in Continuous Quality Improvement (CQI) Program, provides information and reports based on CQI indicators. Distributes and collects patient satisfaction data, relaying the feedback to the concerned sections for purpose of future improvement to better meet the patient's needs. Follows up all incident reports involvina section provides appropriate action and written response.
9.Ensures that safety procedures are implemented and practiced.
10. Carries out assigned projects, recommends new approaches, using systems management methods.
11. Ensures that senior staff of the section can handle the supervisory duty at any time.
12. Follows all Hospital related policies and procedures.
13. Participates in self and others' educational, training and development, as applicable.

**Education**:
Master’s Bachelor’s Degree in Hospital Administration or other related discipline is required.
